It's pretty typical that the front of the case just says something like "RACHMANINOV", but the back will say "Sergei Rachmaninov" or the like. Myself, I prefer the latter.

I don't personally like crediting only the last name. I think reosarevok agrees, but I don't think there are any guidelines spelling out what to do in that case.
