        Sorry for jumping into your conversation, but I'm part of a team that responds to surveys (somewhere around 100 per year). The format that I appreciate the most is when there's a number of "potential" answers to select from (as many as apply), plus a "none of the above", plus a field to enter your own / additional responses. It makes it fairly quick and the responses can easily be summarized and

        I'm also struggling a little with the question about being familiar with the artists. Some I've entered I've exchanged email with them, but have never met them in person. Some my niece's husband knows personally, but I've never met them. I guess both of those would be somewhere in the middle?

        My only comment about the demographics questions is that much of the raw data will need to be processed manually. If you're interested in things like age ranges, make that a radio button collection. Most countries could be selected from a pull-down list, with the ability to select multiples, and a text box for ones not listed.
